Please save your form as a pdf and email it along with:
1) copy of your card front and back 
2) and a copy of your driver's license to [email protected]

There will be several charges on your credit card by Diva Dee Destinations and 2 or 3 other vendors that will equal your total trip price, depending upon the trip that you are booking. Please contact your credit card company to advise them that you are booking your vacation and to  please approve the charges so that your payments will process and your account is not marked for Fraudulent Activity.

Please be advised that some International Countries only accept Master Card and Visa and will have an additional  4% - 5 %  in fees that are not included in your total trip price.
